If you’re gluten-free, this moment is probably familiar:
You’re exhausted.
Your reflux is flaring.
You’re standing in a pharmacy aisle holding Nexium 24HR trying to decide if it’s worth the risk.
And the box doesn’t say “gluten-free.”
But it also doesn’t say it’s not.
That gray area is where most OTC medications live.
This week, we’re breaking down the over-the-counter version of Nexium 24HR specifically, not the prescription version.
If you’d rather skip ahead and browse medications already labeled gluten-free on DailyMed, you can search the database here:
Now let’s unpack Nexium.

Quick Verdict (TL;DR)
Here’s the short version before we unpack it:
❌ Nexium 24HR (OTC) is not labeled gluten-free on the box
⚠️ DailyMed labeling does not state gluten-free status
❌ The manufacturer does not test the finished product for gluten or guarantee gluten-free status
🧪 Independent NIMA testing detected no gluten in the specific OTC Nexium 24HR product tested (NIMA is not validated for medications and results are informational only)
👉 Your comfort level depends on how much certainty you personally need.
If You Prefer a Labeled Alternative:
If you’d rather choose a product that is explicitly labeled gluten-free on the box and reflected in DailyMed documentation, here is one OTC option currently labeled gluten-free:
👉 Amazon Basic Care Esomeprazole 20mg Capsules (Currently labeled gluten-free on the box and reflected in DailyMed at time of review)
(Always confirm labeling before purchase, as formulations may change.)

What The Manufacturer Told Us:
We contacted the manufacturer regarding the gluten content of Nexium 24HR OTC variants.
Here’s their response:
"We can confirm, Nexium products do not contain gluten in the formulation. However, we cannot guarantee that minute amounts of gluten or gluten derivatives are not contained in raw materials obtained from various suppliers for our manufacturing process since the end product is not tested for gluten.
Our products are manufactured in GMP compliant facilities with ingredients tested to ensure they consistently meet the intended quality standards prior to use in our products. We follow procedures based on current Quality and Good Manufacturing Practice (cGMP’s) standards that prevent and control cross-contamination. Also, cleaning practices are in use to avoid contamination from previous raw materials throughout the manufacturing and packaging process. Furthermore, the raw materials and product components are qualified and monitored through a Supplier Management System program and, all materials and products have an identification number that ensures the identity and traceability of each material including materials used in Nexium products.
Consumers with known allergy to gluten or experience gluten intolerance/ celiac disease should consult their doctors for further advice.”
This is a cautious and legally conservative response.
What That Response Actually Means:
“No gluten-containing ingredients” does not mean “gluten-free.”
What Haleon is telling us is:
Gluten is not intentionally added
Gluten testing is not performed
Trace contamination cannot be ruled out
There is no gluten-free guarantee
This is common in OTC medications.
It’s not incorrect. But it’s not a guarantee either.
What I Personally Found in Store:
We reviewed the over-the-counter Nexium 24HR packaging in store.
There is no gluten-free statement on the box.
No front panel claim.
No side panel disclosure.
No gluten mention in the labeling.
That doesn’t mean it contains gluten.
But it does mean there is no explicit gluten-free labeling claim.
‼️Important Nima Disclaimer (Please Read Carefully)
The NIMA gluten sensor:
Is commonly used for food testing
Has not been third-party validated for use with medications or supplements
Cannot detect gluten below its established detection threshold
Does not provide certification or guarantees
Very important:
The NIMA device has not been third-party validated to accurately measure gluten in medications or vitamins. All results are based upon AllerguardRx testing, and NIMA Partners, Inc. has not been involved or consulted in the results.
Because of this:
NIMA results should be viewed as supplemental information only
They do not override manufacturer statements or labeling
They represent one additional data point, not a definitive answer
Individual reactions and sensitivities vary, and results may not reflect every batch or formulation.
*We will always disclose this clearly and never present NIMA testing as proof of safety.
🧪 Independent Testing with Nima (An Additional Data Point):
Because labeling and manufacturer statements don’t always provide full clarity, we performed independent testing on the OTC Nexium 24HR capsule product using the NIMA gluten sensor.
NIMA Test Results
🧪 No gluten was detected in OTC Nexium 24HR 20mg Capsules.

Why We Still Share This Data:
For many gluten-free individuals, especially those who are highly sensitive, more transparent information, when clearly labeled, is better than silence.
Our goal is not to remove all uncertainty.
It’s to provide as many data points as possible, so you can make the decision that’s right for your body.
Why This Isn’t in the AllerGuard GF Database:
You may notice Nexium 24HR is not currently listed as gluten-free in the database.
That’s intentional.
We only include medications when:
✔️ They are labeled gluten-free in official DailyMed / SPL documentation
✔️ The gluten-free claim is clearly reflected in FDA-linked labeling
Right now:
• The box is not labeled gluten-free
• DailyMed does not reflect a gluten-free claim
• The manufacturer does not test or guarantee
So it remains outside the labeled gluten-free database.
Accuracy over assumptions. Always.
What This Means For You:
If you’re gluten-free and considering OTC Nexium 24HR:
• No gluten-containing ingredients are listed in the formulation
• The manufacturer does not test for gluten
• No gluten was detected in the product tested* (See NIMA disclaimer above)
• There is no official gluten-free labeling claim
For some people, that is enough reassurance.
For others, especially those with celiac disease or high sensitivity, the lack of formal labeling may matter.
Your risk tolerance matters.
